Application Requirements
- HHA or PCA Valid Certificate
Proof of your certification as a Home Health Aide or Personal Care Aide.
- Two Reference Letters
Letters from previous employers or professional references. Note: No family members or coordinators.
- Two Picture IDs
Acceptable forms include NY State ID/license, Passport, Green Card, or US birth certificate.
- Direct Deposit Form or Voided Check
- Medical Insurance Card
Proof of medical insurance (if applicable).
Medical Requirements (Must not be older than 9 months)
- Complete Physical Examination
Proof of a recent physical examination.
- QuantiFERON-TB Gold Lab Result or PPD
Proof of a negative QuantiFERON-TB Gold lab result or PPD. If positive, a chest X-ray report with a negative result is required.
- Rubella (German Measles) with Lab Report/Titers
Proof of immunity. If not immune, provide documentation of one MMR vaccine.
- Rubeola (Measles) with Lab Report/Titers
Proof of immunity. If not immune, provide documentation of two MMR vaccines.
- Drug Screening with Attached Lab Report
Proof of a recent drug screening (negative result, at least 7-panel test).
- Covid Vaccination Card
Proof of COVID-19 vaccination (if applicable).
